Any athlete with incomplete physical forms, including the lack of proper signatures, will not be cleared until completed and reviewed by the nurse.
 
THERE WILL BE NO EXCEPTIONS! COMPLIANCE WITH THESE FORMS ARE REQUIRED BY THE HIGH SCHOOL AND THE STATE OF NEW JERSEY

 The following attachment is the listing of Banned Substances according to NJSIAA:

Any athlete who has had a physical that is more than 60 days old must fill out the following Medical History Update form in order to be cleared for sports.

Example: If the physical was done in August for a Fall sport and now the athlete is planning to play a Winter sport, the Medical History Update form MUST be completed

IMPACT BASELINE TESTING (CONCUSSION TESTING) - 2011

The state has made Impact Concussion Testing mandatory this year.  This test will help the Athletic Trainer evaluate your child in the future if ever the need arises. The baseline test is good for 2 years so if they took the test last year, they do not have to take it again until next.  You will need to download the Concussion and Acknowledgement Form as part of the Physicals packet in order to be cleared by the nurse. 

Please take some time to download and read the following Parent/Coach Communication Guide. It will answer most questions you may have regarding contact with your child's coach or the athletic director.  Please be sure to follow the guidelines on this document when communication is needed.
